This form is for residents / visitors / HC Employees attending HCOCA/TD Events to request any accessibility services for upcoming HCOCA/TD Events ONLY. For other non-HCOCA/TD accessibility service requests for Hudson County please go to Hudson County Health & Human Services – Office of inclusion and Accessibility website.

Two weeks advanced notification required. Our Office will make every effort to provide your requested service for the Event listed, however, it is not guaranteed. We will contact you if additional information is needed.
