Assistive Listening System (ALS) Request Form for Organizations

*PLEASE READ BEFORE SUBMITTING FORM*

If you need audio description, you will need to hire a separate audio describer to provide that service using the equipment included – see resource list for audio describers. There are 2 audio description headsets included in the system. All equipment is contained in a special case and is expected to be returned promptly in the same condition as it was received. 

The person picking up the system will be required to sign in and sign out.  Your organization will be responsible if the system is damaged/lost/or missing components. 

Two weeks advanced notification required.  Our Office will make every effort to provide your requested service for the Event listed, however, it is not guaranteed.  We will contact you if additional information is needed.
